A 1,200-pound (543.6 kilogram) gate fell on a worker, trapping him between his pickup truck and the gate and asphyxiating him.
He was a maintenance worker, leaving work for the day. The 40- foot (12.91-meter) by six-foot (1.83-meter) gate was secured in the open position by a long chain. Several people at the worksite had informed the victim about the inoperable condition of the gate and that it was chained open for maintenance — to replace rollers and other hardware. Despite their warnings, the victim removed the chain and tried to close the gate manually. It fell and trapped him.
Your attitude can kill you. In this case, the victim may have felt complacent when he disregarded the hazard warnings. No one will ever know.
