Adapt this form for your OHS program and the requirements of applicable OHS laws on PPE. Use it to document that you’ve provided fall protection training to workers, what that training covered and that the workers understood their training and demonstrated how to use the equipment on which they were trained. Retain these completed forms in the event your fall protection training is questioned.
