Hallan a un Trabajador Fallecido dentro del Tanque de Agua de un Camión de Vapor (Alberta, Canadá — 19 de febrero de 2022)

Resumen del Incidente

El 19 de febrero de 2022, se encontró sin vida a un trabajador del sector de la minería y el desarrollo petrolero de Alberta dentro del tanque de agua de un camión de vapor. El trabajador había estado realizando tareas de mantenimiento o inspección por su cuenta. Debido a que no había ningún compañero de trabajo presente y no se contaba con un sistema de registro de presencia en el momento del incidente, la ausencia del trabajador no se detectó a tiempo para intentar un rescate.

Investigación

La Oficina de Salud y Seguridad Ocupacional de Alberta (OHS) abrió una investigación sobre la muerte en el lugar de trabajo con el expediente F-OHS-269391. La investigación examinó la evaluación de riesgos, los procedimientos para trabajar solo, los controles de entrada a espacios confinados y la respuesta de emergencia. El caso fue remitido para la revisión de medidas coercitivas a fin de determinar el enjuiciamiento en virtud de la Ley de Salud y Seguridad Ocupacional de Alberta.

Riesgos clave Demostrados por el Incidente

El caso refleja tres patrones que el NIOSH y los reguladores canadienses de OHS citan repetidamente en los casos de muertes de trabajadores solitarios. En primer lugar, el ingreso en espacios confinados en solitario —incluso por un breve periodo de tiempo— deja a nadie disponible para reconocer una situación de peligro, dar la alarma o realizar un rescate. En segundo lugar, la ausencia de un sistema verificado de comunicación bidireccional retrasó el descubrimiento más allá del margen de supervivencia. En tercer lugar, las evaluaciones de riesgos para trabajadores solitarios en sitios remotos y del sector de recursos a menudo subestiman el tiempo necesario para que la respuesta de emergencia llegue al lugar.

Conclusiones

Nunca entre solo en un espacio confinado, por muy rutinaria que parezca la tarea. Establezca un horario de control con confirmación bidireccional verificada y un procedimiento de escalamiento documentado en caso de que se omita un control. Realice una evaluación de riesgos por escrito para cada tarea de un trabajador solitario y actualícela cada vez que cambien las condiciones. Equipe a los trabajadores solitarios con un dispositivo de comunicación confiable, energía de respaldo y una función de pánico/SOS adecuada para el sitio
