The Moral: Any floor holes big enough for a person to fall into must be guarded by either a standard railing with standard toeboard on all exposed sides or a floor hole cover of standard strength and construction (OSHA, Guarding Floor & Wall Openings & Holes, Sec. 1910.23(a)(8)). A mere plank of wood won’t suffice!
