Un equipo acaba de recibir una asignación de tareas y se apresura a comenzar a trabajar en un equipo recientemente modificado. Las herramientas ya están preparadas, un supervisor está ocupado hablando por teléfono y un formulario de evaluación de riesgos previa a la tarea se encuentra sobre una mesa cercana —en blanco y sin firmar—. El equipo no ha sido inspeccionado para detectar nuevos riesgos, no se ha verificado que las fuentes de energía estén aisladas y no se ha informado a los trabajadores sobre lo que podría salir mal. Un trabajador ya se encuentra en una zona que sería peligrosa si el equipo se activara inesperadamente.

En entornos de trabajo de ritmo acelerado, la presión por comenzar rápidamente es una de las razones más comunes por las que se omiten o se realizan a toda prisa las evaluaciones de riesgos; sin embargo, las tareas no evaluadas no se vuelven más seguras una vez que comienza el trabajo, sino que se vuelven más peligrosas. Antes de comenzar cualquier tarea, detente y tómate el tiempo para identificar qué podría salir mal en cada paso y comunica los riesgos y las medidas de control a todas las personas involucradas. Una evaluación de riesgos no tiene por qué llevar mucho tiempo, pero sí es necesario realizarla. Esos pocos minutos podrían ser los más importantes de la jornada laboral.
