A fall down an elevator shaft killed a construction supervisor. He and his crew were setting up a work platform in an open elevator shaft when a board supporting the platform broke and he fell 33 feet (ten meters) to the concrete floor. He wasn’t using any fall protection equipment.
Is it possible that you’ll be exposed to a fall hazard? If so, use appropriate fall protection equipment. Make sure walking/working surfaces are strong and structurally sound enough to support people. Have you had training in hazard recognition and use of fall protection devices?
