OSHA claims employees were exposed to the dangers of falls, crushing injuries, burns, hearing loss, lacerations, amputations and electrocution. Among the alleged hazards were defective forklifts, employees working under raised or unstable loads, wet floors, clogged floor drains, missing guardrails, lockout issues, damaged lifting slings and unguarded machinery. Proposed penalties totaling $117,250 have been issued.
