A metal treatment company was investigated by OSHA after an employee died in a fire in November 2007. OSHA says it found a number of workplace safety violations involving electrical hazards, unsafe handling of flammable liquids and vapors, failure to address respirator needs and hazards, and failure to protect employees against exposure to hexavalent chromium. The citations carry $121,800 in proposed penalties.
