An ironworker was standing on top of a large door which was to be attached to hinges on a building under construction. The door had been raised by a crane. After the crane lifted the door, the weight of the door was then taken up by a pulling device used to put it in place. The pulling device failed, and the door fell. The worker fell 50 feet and died later at a hospital.
Don’t ride a suspended load. A backup support should have been provided to suspend the load. The worker should also have been wearing fall arrest equipment.
