| What You NEED To Do |
| When your operators have passed their exam they will be required to go through a live forklift operator evaluation using the form called Forklift Operator Evaluation. The instructor will complete the evaluation form for each operator while he/she observes and checks that the operator successfully performs each applicable item listed.
It is recommended that you set up a small forklift field course in your facility that corresponds with the forklift operator evaluation. This will assist you in better evaluating your operator and your operator will be able to better demonstrate operational proficiency. When the evaluation form is completed, make sure the instructor and operator signs and dates it.
