OSHA conducted an investigation at a Covington, LA, worksite in March 2008 and issued three repeat citations concerning alleged failure to protect employees from fall hazards. OSHA says the company failed to provide employees with fall hazard equipment while performing steel erection activities and did not provide training and instruction to employees exposed to fall hazards. Additionally, OSHA alleges that the company failed to ensure that a scaffold platform used to lift workers was attached to the forks of a rough terrain forklift. Proposed penalties total $72,000.
