In a devastating incident at Disston Elementary School in Philadelphia, a five-year-old kindergarten student, Jonathan Cozzolino, lost his life when a folded cafeteria table tipped over and fell on him. The table, which was stored in an upright position, became unstable and collapsed as Jonathan was leaving the gymnasium after lunch. He suffered fatal head trauma from the accident.
Investigations revealed that the school district possessed approximately 500 similar tables, many of which were found to be faulty. The U.S. Consumer Product Safety Commission had previously issued warnings about the dangers of portable cafeteria tables tipping over, emphasizing the need for proper storage and supervision.
