OSHA inspected a carnival located at a mall in Bensalem, PA and issued a willful citation alleging failure to provide employees with adequate fall protection. A separate inspection at another site resulted in two serious citations being issued after an employee fell from the top of a carnival ride. The citations, which carry total proposed penalties of $62,000, allege lockout/tagout deficiencies that led to inadvertent startup of equipment.
