OSHA launched an investigation after an employee died and two other workers were injured in separate incidents a few weeks apart. The company was cited for one willful and nine alleged serious violations relating to electrical hazards. The willful citation alleges failure to ensure the use of appropriate electrical protective equipment when employees were working on or near energized electrical parts. According to OSHA, workplace conditions contributed to two employees being burned by an electrical arc flash April 9 and adequate safety measures were not in place to protect against electrical hazards at the time of the worker’s death on Feb. 2, 2009. OSHA has issued $128,500 in proposed fines.
