A roofing contractor has been cited for five alleged repeat violations after an OSHA inspector observed employees working without fall protection at heights of 38 to 44 feet. The contractor was cited for allowing employees to work without fall protection, failure to train employees in fall protection methods and fall hazards, failure to provide a safety monitor competent in identifying fall hazards, failure to position a safety monitor to communicate with employees about fall hazards and failure to ensure that an employee wore heat protection. The company had been cited for similar hazards at other worksites in September 2006 and April 2007. Penalties of $70,000 have been proposed.
