After receiving a complaint alleging electrical and machine guarding hazards, OSHA initiated an investigation on Sept. 27, 2006. As a result, two willful violations and 10 serious violations were cited. Willful violations were issued for alleged failure to properly guard machinery. Serious violations being alleged include failure to establish procedures to ensure the control of hazardous energy from machines, failure to train employees in the safe operation of powered industrial trucks, and electrical hazards. Total penalties of $111,500 have been proposed.
