Citations were issued for four alleged willful, repeat and serious violations of safety standards following an OSHA inspection at a water main installation site in September 2006. Employees were found working inside a nine-foot-deep trench that lacked protection against a collapse of its sidewalls. Willful citations were issued because the company had been cited for the same hazard just one month earlier. Proposed penalties total $64,700.
