Trabajador Muerto en Destello de Arco y Explosión en Subestación de Eversource Energy

Resumen del Incidente

El 12 de julio de 2022, Fabio Pires, empleado de Eversource Energy de 31 años, murió a causa de un destello de arco y una explosión mientras trabajaba en equipos eléctricos energizados en Boston. El arco ocurrió cuando el equipo se volvía a colocar en su lugar durante el mantenimiento. Pires sufrió lesiones térmicas y por explosión fatales en el lugar.

Investigación

OSHA abrió una inspección y propuso multas por un total de $333,560 contra Eversource Energy. Los investigadores encontraron que la empresa no había desenergizado completamente el equipo, no había seguido las recomendaciones de mantenimiento del fabricante y no había realizado una estimación razonable de la energía calorífica (energía de incidente) a la que estarían expuestos los trabajadores si ocurriera un destello de arco. Se emitieron citaciones bajo los estándares federales de prácticas de trabajo seguro en sistemas eléctricos y de EPP.

Peligros Clave Demostrados por el Incidente

El caso refleja los tres fallos que OSHA cita con mayor frecuencia en las fatalidades por destello de arco. Primero, se realizó trabajo en equipo que debería haber sido desenergizado y bloqueado bajo 29 CFR 1910.147. Segundo, el empleador no realizó un análisis de energía de incidente ni especificó EPP con clasificación de arco adecuada al peligro, conforme a NFPA 70E. Tercero, los procedimientos de mantenimiento del fabricante no se siguieron durante el reensamblaje, lo que permitió la falla que inició el arco.

Conclusiones

Desenergiza, bloquea, etiqueta y verifica que está desenergizado con un medidor conocido como bueno antes de cualquier tarea de mantenimiento. Realiza una evaluación de peligros de destello de arco y define la energía de incidente y los límites de aproximación para el equipo. Usa el EPP con clasificación de arco que la tarea requiere, siempre. Sigue los procedimientos de mantenimiento del fabricante exactamente durante el reensamblaje y la re-energización.
