OSHA conducted an inspection at a west Milwaukee plant in September 2006 after receiving information alleging indoor air quality concerns and personal protective equipment hazards. The company was issued one serious and four repeat citations alleging a lack of guarding on chain and sprocket equipment, a lack of guarding on mechanical presses and load rams, failure to follow lockout/tagout procedures and a lack of mechanical power press inspections. Three serious health violations were also cited. They allege a lack of PPE during exposure to cutting fluids, a lack of proper labeling on hazardous chemicals and blocked space around electrical panels. The company also received one other-than-serious citation alleging that aisles were not properly marked. The company has been inspected by OSHA 24 times and has received 71 safety and health citations.
