A worker was fatally injured while grinding on the lid of a metal toolbox. The sparks from his grinder ignited an explosive mixture of oxygen and acetylene which had leaked from a torch placed inside the tool box.
The explosion caused the lid of the toolbox to fly upward, which in turn caused the grinder to hit the worker’s head.
Explosions can happen when you don’t check out your environment before starting a task. Watch for hidden hazards.
