A supervisor at a ship repair dry-dock was crushed to death by a large plate of steel as it was being lowered.
He and his crew had been repairing the bottom of a ship. They had cut around the damaged area and were preparing to lower a piece of steel. The victim was overseeing the operation and gave the all-clear signal. Nobody knows why, but he stepped back into the area below the steel plate. It dropped and hit him.
A moment of bad judgment on a job such as this can be fatal. Make sure you always follow the safe procedures for your job so you will not make a hasty decision to step into a hazardous situation.
