A driver working for a shiploader was killed when he backed a forklift off the edge of a pier. There was no barrier or bull rail in place to prevent such a tragedy, but a concrete barricade has been installed since the fatality.
Many fatalities have occurred when forklifts travel over the edge of loading docks and other elevated surfaces. Another common scenario for fatalities is when a forklift is unloading goods from a truck trailer. The vibration causes the truck to creep away from the loading dock, and the forklift falls through the gap between the dock and the truck gate.
