OSHA has issued citations for numerous alleged hazards relating to inadequate safeguards involving formaldehyde and various chemical, mechanical and electrical hazards. OSHA says the plant lacked a process safety management program and procedures to proactively assess hazards associated with processes and equipment using large amounts of formaldehyde. It also lacked controls and other safeguards to reduce the levels of formaldehyde to which some workers were overexposed. OSHA also identified hazards related to respiratory protection, emergency response, hazardous energy control, chemical hygiene, chemical hazard communication, electrical issues, machine guarding, untrained forklift operators, use of unapproved forklifts around flammables and unclean work floors. Fines totaling $138,000 have been proposed.
