The policy can cover substance use issues, or it can use an overall approach such as impairment in the workplace. The main goal is that workplaces are encouraged to establish a procedure or policy so that help can be provided in a professional and consistent manner. It is important for supervisors and managers to have a resource or procedure that they can rely on if the need arises. Employees need to know that everyone will be treated the same way. These actions help to reduce the stigma associated with substance use. When stigma is reduced, it is hoped that people will seek help without fear, and will speak openly about substance use issues. Early treatment and support is encouraged.

Managers and Supervisors should be educated in how to recognize and deal with substance use issues and employees should be offered educational programs. Note it is not the role of the supervisor or employer to diagnose a possible substance use or dependency problem. Their role is to identify if an employee is impaired, and to take the appropriate steps as per the organization’s policy.

Training/Prohibitions

  • Train supervisors to monitor employees carefully. Share drug prevention information and hang posters with OSHA drug prevention guidelines.
  • Prohibit alcohol on the premises to discourage drinking and make it known that you’ll schedule periodic, unannounced drug testing. Employees will be far less likely to use drugs, knowing they could be tested at any time and lose their job.

He had arrived at work with an apparent hangover. Co-workers had not seen him very much that day because he had reportedly spent most of the shift in an enclosed van.

An explosion in the van caused second and third degree burns to 90 per cent of his body. An oxy-acetylene torch with an open valve, an oxy-acetylene tank with an open regulator and an open tank of gasoline were found in the van. It is assumed the torch set off an explosion of gasoline vapors. A syringe with traces of cocaine and blood matching the victim were also found. Fellow workers told investigators the victim had exhibited unusual behavior for the last six months and had been hiding cocaine in the van.

Drug use at work can be fatal, affecting your skills and judgment. If fellow workers are showing signs of drug abuse, it’s your responsibility to report it before someone gets hurt. The hazards of drug abuse and what to do about it should be part of a regular safety training program. When flammable liquids or gases are transported, they must be in approved and appropriately ventilated containers.

Cold weather and environments pose many hazards to employees who work in these conditions. A cold environment forces the body to work harder to maintain its temperature.

WHAT’ S THE DANGER

HYPOTHERMIA

When exposed to cold temperatures, your body begins to lose heat faster than it can be produced. Prolonged exposure to cold will eventually use up your body’s stored energy. The result is hypothermia, or abnormally low body temperature. A body temperature that is too low affects the brain, making the victim unable to think clearly or move well. This makes hypothermia particularly dangerous because a person may not know it is happening and will not be able to do anything about it.

Symptoms

Symptoms of hypothermia can vary depending on how long you have been exposed to the cold temperatures.

Early Symptoms

  • Shivering
  • Fatigue
  • Loss of coordination
  • Confusion and disorientation

Late Symptoms

  • No shivering
  • Blue skin
  • Dilated pupils
  • Slowed pulse and breathing
  • Loss of consciousness

FROSTBITE

Frostbite is an injury to the body that is caused by freezing. Frostbite causes a loss of feeling and color in the affected areas. It most often affects the nose, ears, cheeks, chin, fingers, or toes. Frostbite can permanently damage body tissues, and severe cases can lead to amputation. In extremely cold temperatures, the risk of frostbite is increased in workers with reduced blood circulation and among workers who are not dressed properly.

Symptoms of frostbite include: Reduced blood flow to hands and feet (fingers or toes can freeze), Numbness, Tingling or stinging, Aching, Bluish or pail, waxy skin.

Trench Foot

Trench foot, also known as immersion foot, is an injury of the feet resulting from prolonged exposure to wet and cold conditions. Trench foot can occur at temperatures as high as 60 degrees F if the feet are constantly wet. Injury occurs because wet feet lose heat 25-times faster than dry feet. Therefore, to prevent heat loss, the body constricts blood vessels to shut down circulation in the feet. Skin tissue begins to die because of lack of oxygen and nutrients and due to the buildup of toxic products.

Symptoms of trench foot include: Reddening of the skin, Numbness, Leg cramps, Swelling, Tingling pain, Blisters or ulcers, bleeding under the skin, Gangrene (the foot may turn dark purple, blue, or gray).

HOW TO PROTECT YOURSELF

BODY REACTS TO COLD

In a cold environment, most of the body’s energy is used to keep the internal core temperature warm. Over time, the body will begin to shift blood flow from the extremities (hands, feet, arms, and legs) and outer skin to the core (chest and abdomen). This shift allows the exposed skin and the extremities to cool rapidly and increases the risk of frostbite and hypothermia. Combine this scenario with exposure to a wet environment, and trench foot may also be a problem.

EMPLOYEE PREVENTION

Workers should avoid exposure to extremely cold temperatures when possible. When cold environments or temperatures cannot be avoided, workers should follow these recommendations to protect themselves from cold stress:

  • Wear appropriate clothing.
    • Wear several layers of loose clothing. Layering provides better insulation.
    • Tight clothing reduces blood circulation. Warm blood needs to be circulated to the extremities.
    • When choosing clothing, be aware that some clothing may restrict movement resulting in a hazardous situation.
  • Make sure to protect the ears, face, hands and feet in extremely cold weather.
    • Boots should be waterproof and insulated.
    • Wear a hat; it will keep your whole body warmer. (Hats reduce the amount of body heat that escapes from your head.)
  • Move into warm locations during work breaks; limit the amount of time outside on extremely cold days.
  • Carry cold weather gear, such as extra socks, gloves, hats, jacket, blankets, a change of clothes and a thermos of hot liquid.
  • Include a thermometer and chemical hot packs in your first aid kit.
  • Avoid touching cold metal surfaces with bare skin.
  • Monitor your physical condition and that of your coworkers.

  • When working outdoors in cold weather or working in artificially cold environments, such as refrigerated areas, serious cold-related injuries and illnesses may occur. Cold related hazards can cause permanent tissue damage or even death.
  • Snow shovelling is particularly strenuous because it uses arm work, which is more taxing than leg work. Straining to move wet and heavy snow is particularly likely to cause a surge in heart rate and blood pressure.
  • Many people hold their breath during the hard work, which also puts a strain on the body.
  • People at greatest risk are those who are habitually sedentary with known or suspected coronary disease, who go out once a year to clear snow, increase the risk. If you must do it, push rather than lift the snow, dress in layers, take regular breaks indoors and don’t eat or smoke before shovelling.

  1. Every winter, about 100 people in the US die while shovelling snow.

A study looking at data from 1990 to 2006 by researchers at the US Nationwide Children’s Hospital recorded 1,647 fatalities from cardiac-related injuries associated with shovelling snow. In Canada, these deaths make the news every winter.

  1. CDC Report: Cold Kills More People Than Heat

A report by the National Center for Health Statistics reveals that winter cold kills more Americans than summer heat. Each year, approximately 2,000 workers die due to weather-related causes. Of that population, 63% of deaths are caused by exposure to cold and/or hypothermia, while only 31% are attributed to heat-related exposure such as heat or sun stroke.

  1. According to the Bureau of Labor Statistics, in 2014, there were 42,480 workplace injuries involving ice, sleet, or snow that demanded at least one day off. Of these injuries, and illnesses, 34,860 (82 percent) were the result of falls on the same level – not from heights.



Cold Stress Hazards Fatality Report

A 69-year-old FedEx worker has been found dead outside a delivery hub in western Illinois, and police believe the extreme cold snap that hit the country this week played a role.

The man, whose name wasn’t released, is among 23 people killed in the United States as a result of extreme weather conditions this week. Cook County, Illinois, which includes Chicago, announced three deaths on Friday, including a 92-year-old man.

At the FedEx delivery hub in East Moline, a coworker found the man’s body outside between two tractor-trailers Thursday morning, police Capt. Darren Gault said.

It wasn’t immediately clear how long the worker had been there, or why he was there. The hub had been closed on Wednesday.

It was extremely cold in the Moline area this week. At an airport only 10 miles from the hub, temperatures dipped below zero late Tuesday afternoon and never rose above that until Thursday evening. Wednesday evening’s low was 29 below with a wind chill of 34 below; Thursday morning’s low was 33 below.

“We are saddened by the loss of our team member and our sympathies go out to his family and friends,” FedEx said in a statement Friday. “We are working with local authorities as they investigate.

“FedEx has contingency plans in place to ensure the well-being of our team members and service providers. Several of our centers, including the Rock Island center, were closed or running very limited operations this week due to the extreme cold.”

Inexperienced and young workers remain a vulnerable group in all industries, in particular the construction industry, as demonstrated by the recent sentencing hearing of a concrete pump contractor in SafeWork NSW v NonAbel Concrete Pump Pty Ltd; Bilal Hamdan (2020).

NonAbel Concrete Pump Pty Ltd (Company) is a company providing concrete pumping and placements services. The Company owned a concrete pumping truck, which was fitted with a delivery boom and other equipment, such as concrete delivery hose and pipeline components. The truck was in use for a construction of 5-storey residential apartment at 62–64 Wrentmore Street, Fairfield NSW (worksite).

Ware Building Pty Ltd (Ware) was the principal contractor that engaged S.E.P. Formwork Solutions Pty Ltd (S.E.P.) as a subcontractor to provide concreting works at the worksite. S.E.P. then engaged the Company as a subcontractor.

Mr Bilal Hamdan was the sole director, secretary and shareholder of the Company, and one of the Company’s two employees. The other employee was Mr Souhayb Jamal, who was engaged as a labourer and concrete pump linesman. The Company employed Mr Jamal for between only 3 to 7 days before the incident.

On 27 March 2017, the Company was using the concrete pumping truck to place concrete on the level 5 formwork deck. A metal pipe was attached to the delivery boom of the truck and a 10.2-metre black rubber concrete delivery hose was attached to the end of the metal pipe. The end of the hose was fitted with a metal coupling.

Before the incident, Mr Franco Vellar, who was employed by Ware as a site foreperson, saw Mr Jamal for the first time at the worksite office. Mr Vellar started talking to Mr Jamal, but it was apparent that Mr Jamal’s English was poor. Mr Vellar had partially completed an induction form for Mr Jamal when he received a call and left to assist on site. He instructed Mr Jamal to remain seated in the office. However, shortly afterwards, Mr Jamal left the office and went to level 5 without notifying Mr Vellar.

A short time later, Mr Hamdan, who was trying to pump concrete, realised that there was a blockage in the concrete delivery hose. He stopped and asked a crane operator to help. After detaching the hose from the fixed pipe, the crane operator lifted the hose using the crane while Mr Hamdan and Mr Jamal hit the hose with a hammer in an attempt to clear the blockage. The hose was then reattached, and Mr Jamal walked back to the end of the concrete delivery hose, stood with the hose between his legs and picked up the end of the hose with his hands.

Mr Hamdan then performed a blowout with air pressure to clear the blockage in the concrete delivery hose. The concrete delivery hose whipped around and struck Mr Jamal in the head, causing him to flip over and land on the ground. Mr Jamal was not wearing a safety helmet. Mr Jamal died from the severe head injuries.
