Hoisting is used to lift and lower loads using a drum or wheel with ropes or chains wrapped around it. Hoists can be powered manually, electrically, or pneumatically.

Only workers who have been trained in the proper use of hoists should be allowed to operate them. The lifting capacity of the hoist must be clearly marked and visible to the operator and cage-controlled hoists must be equipped with effective warning devices. Before operation, operators should check that the hoist chains or ropes are of sufficient strength and length to safely lift or otherwise handle the load. On a chain hoist, they should make sure the hook has a safety clip so that if the chain is given slack the hook won’t come loose. The oil level on hydraulic hoists should also be periodically checked. Operators should understand that they are prohibited from carrying loads over people and any hoist malfunction should be reported to their supervisor immediately.
HOISTING EQUIPMENT HAZARDS
When a hoist or load inadvertently collides with another object or stationary structure, it can swing wildly, cause severe impacts, or fall into an unsafe area. The accident may not only hurt the workers operating the equipment, but it can also hurt workers nearby on board a ship, vessel, dock, or oil or natural gas platform. Workers in these conditions can be trapped, struck, crushed, or knocked off the vessel by an inbound load or lifting equipment.
A hoist should be strong and stable, and equipped with the suitable ropes and fittings, to ensure the safety of employees using it and of those working in the vicinity. Like any other piece of equipment, there are a number of safety hazards associated with their use. Some examples are:

A Competent Signaller – Selection
Designate a competent employee to be a signaller – to direct, by means of visual or auditory signals, the safe movement and operation of a hoisting apparatus by an operator.
To operate a hoist, you must be properly trained. Know the rated capacity of your hoist; it should be clearly labeled on the equipment. Read the manufacturer’s operating instructions and warnings. Get training on how to use the hoist machine and how to properly rig and safely maneuver loads. Perform regular maintenance on the hoist and lifting rope/chain.

COMMON CAUSES OF HOISTING EQUIPMENT ACCIDENTS

A machine operator was killed while operating a palletizing machine at an ice cream manufacturing facility. The palletizer was designed to place and stack products from an assembly line onto pallets for shipping. The incident occurred after the victim had finished working a full second shift and was in the midst of four hours of volunteered overtime. The victim entered the machine’s hoist area to position a pallet when the hoist unexpectedly began to rise. The machine was operating in automatic mode, which allowed the palletizer to continuously cycle. It was not locked out. A co-worker heard cries for help and ran to stop the machine. The victim was crushed between the empty pallet and the top portion of the hoist. When workers began to lower the hoist, the victim fell to the ground and was pulled away from the machine. A co-worker who also was an emergency medical technician tended to the victim until emergency medical services arrived. The victim was breathing but was unconscious and unresponsive. He was transported to a local hospital where he was pronounced dead later that day.
To prevent future occurrences:
Minimize the number of times palletizer cycles are interrupted because of failure to detect pallets in the hoist area. To reduce the number of times the palletizer automatically stops because of an undetected pallet, employers should inspect pallets regularly and repair or remove from service those that are damaged.
Employers should ensure all movable parts of machines are guarded at all times to minimize employee access. Three sides of the palletizer involved in this incident were guarded by a cage. The fourth was guarded by a light curtain presence-sensing device. However, the light curtain routinely deactivated between the time a completed pallet exited and a new pallet was properly positioned.
Develop, implement and enforce a comprehensive hazardous energy control program, and routinely review the program and employee training. A hazardous energy control program that includes procedures for lockout/tagout is required by OSHA. All forms of energy must be considered – electrical, hydraulic, pneumatic and mechanical. The program should be enforced by performing random inspections to verify all employees are adhering to safe practices. Training should be conducted at least once a year or when safety concerns arise.

To prevent broken glass from causing laceration injuries, you should make sure that the windows in your garage are made from shatterproof glass.
Most modern garage doors that are less than 10 years old already feature shatterproof glass. If your glass is not shatterproof, have the windows replaced as soon as possible.

A saw can cut the door into smaller, manageable pieces for you to carry. If the old door is paneled, disassemble the panels one at a time to break up the load you will have to carry. Lift with your legs while keeping your back straight and your head looking forward.
When you install the rail system and hardware, avoid long overhead reaches by using a ladder or stepstool. Stay below the top two rungs and move the ladder close to your work. Do not lean to the side on the ladder. Make sure that the weight of you, plus your tools and job materials does not exceed the ladder’s rated capacity.
To install panel hardware, use a sawhorse to position the work to a comfortable height. Kneeling on your knees and/or bending to the ground can lead to strains and sprains. When tracking the door, hook the panels one side at a time securely so they don’t come loose as the door is raised.
Spring winding can be extremely dangerous. Only use the proper winding bars to wind the spring. Never use vise grips, pipe wrenches, socket extensions, screwdrivers, rebar, or other tools. Do not touch any spring set screw without a winding bar in place. Never stand in front of the springs when winding. Stand on the ladder off to the side so that if a bar slips or a casting break, the spinning spring won’t force the bar into you.
Use a ladder to install the automatic opener and ceiling brackets. Falls from ladders are often caused by electric shock. Ensure that the electricity is off before you add the electric outlet or directly wire the opener.

On December 13, 2004, a 62-year-old maintenance mechanic employed by a facility maintenance firm, was fatally injured while servicing an overhead garage door for a shipping company. The facility maintenance firm had been contracted by the shipping company to repair the garage door.
At the time of the incident, the victim and a co-worker were testing the door when the door became jammed with the bottom of the door three feet above the ground. The victim was working on the platform of a scissors lift that was extended approximately 15 feet above the ground and parked parallel to the door. No additional fall protection was used nor is it required by the Occupational Safety and Health Administration (OSHA) when operating a scissors lift with a standard guardrail system. The victim first tried to manually disconnect the garage door arm assembly from the track with the emergency release handle by pulling the handle and cord assembly, but it would not release. He then asked the co-worker for a screwdriver. The co-worker and a shipping company mechanic were both in the area on the ground. The mechanic provided a screwdriver to the victim.
The victim used the screwdriver to pry the emergency disconnect away from the chain drive assembly. According to the co-worker, both he and the victim anticipated that the door would go down upon being manually released due to its weight. Instead the door abruptly sprang upwards to the fully open position, striking the guardrail of the scissors lift and causing the lift to topple to the concrete floor. The co-worker and shipping company mechanic ran to the victim who had fallen out of the scissors lift and was lying on the concrete unresponsive. The mechanic called 911. The EMS squad and police arrived within minutes. The victim was quickly transported to a hospital where he was pronounced dead.
El 13 de diciembre de 2004, un mecánico de mantenimiento de 62 años de edad, empleado de una empresa de mantenimiento de instalaciones, resultó mortalmente herido mientras reparaba una puerta de garaje para una empresa de transportes. La empresa de transporte había contratado a la empresa de mantenimiento para reparar la puerta del garaje.
En el momento del incidente, la víctima y un compañero de trabajo estaban probando la puerta cuando ésta se atascó con la parte inferior de la puerta a un metro del suelo. La víctima estaba trabajando en la plataforma de un elevador de tijera que estaba extendido a unos 15 pies del suelo y aparcado en paralelo a la puerta. No se utilizó ninguna protección adicional contra caídas ni la exige la Administración de Seguridad y Salud en el Trabajo (OSHA) cuando se utiliza un elevador de tijera con un sistema de barandilla estándar. La víctima intentó primero desconectar manualmente el brazo de la puerta del garaje del carril con la palanca de desbloqueo de emergencia tirando del conjunto de palanca y cuerda, pero no se soltó. Entonces pidió un destornillador a su compañero de trabajo. El compañero de trabajo y un mecánico de la empresa de transportes se encontraban en la zona, en el suelo. El mecánico proporcionó un destornillador a la víctima.
La víctima utilizó el destornillador para separar la desconexión de emergencia del conjunto de transmisión por cadena. Según el compañero de trabajo, tanto él como la víctima esperaban que la puerta bajara al soltarla manualmente debido a su peso. En lugar de ello, la puerta saltó bruscamente hacia arriba hasta la posición de apertura total, golpeando la barandilla del elevador de tijera y haciendo que el elevador cayera al suelo de hormigón. WHAT IS FATIGUE?
Fatigue can be broadly defined as a feeling of weariness, tiredness or lack of energy. Fatigue is a common complaint but, medically speaking, it’s recognized more as a symptom or cause of other conditions than as a condition itself. The best way to understand fatigue is along a continuum. On one end of the spectrum is the fatigue that most of us occasionally experience in the course of our lives when we get physically or mentally overburdened. This kind of fatigue isn’t serious and can usually be resolved simply and quickly, such as by getting extra rest. On the other end is a less common but more serious form of fatigue that’s symptomatic of a more chronic and disabling condition, such as major depressive disorder or chronic fatigue syndrome. This form of fatigue is an acute and/or ongoing state of tiredness that leads to mental or physical exhaustion and prevents people from functioning as usual.
HARMS/DANGERS OF FATIGUE
The risk of making mistakes at work increases dramatically if workers sleep for less than seven to eight hours, or are awake for more than 17 consecutive hours.
Fatigue reduces your ability to make decisions and to do complex planning.
Fatigue slows your reaction time, and your ability to respond to changes in surroundings or information provided.
Fatigue lowers your productivity and performance and your ability to handle stress.
Fatigue lowers your overall well-being and increases the risks for illnesses and diseases.
Fatigue can also lead to involuntary micro-sleeps, or brief periods of unconsciousness, that can last two to thirty seconds. These episodes can mimic the effects of being under the influence of alcohol. Eighteen hours without sleep is similar to the effects of a blood alcohol concentration (BAC) of 0.05, while 24 hours without sleep is similar to a BAC of 0.10. These effects can be especially dangerous when operating machinery or driving.

Fatigue can leave workers in a fog – which isn’t a good atmosphere for clear, safe decision-making. The crash of a 747 cargo jet at Halifax International Airport in mid-October 2004 is being blamed in part on flight crew fatigue.
Seven MK Airlines Ltd. crew members died when the big jet failed to gain sufficient altitude on takeoff, struck a berm at the end of the runway and crashed into an adjacent forest.
A Transportation Safety Board of Canada (TSB) investigation found that the speed and thrust settings selected by the crew in preparation for a flight to Spain were incorrect for the weight of the aircraft. The TSB says the crew did not receive adequate training on a computer program used to calculate takeoff velocity and necessary power for flight.
As a result the airplane was traveling about 30 knots (35 miles an hour or 56 kilometers an hour) slower than the 160 knots that would have been required for a safe takeoff with the load it was carrying.
Crew fatigue and a dark takeoff environment contributed to the crash, according to the board. It is calling for implementation of an on-board takeoff performance monitoring system “to ensure that crews of large aircraft will be alerted in time when there is not enough power to take off safely.”
Such a device does not currently exist. Since the airplane’s flight voice recorder was destroyed in the crash, investigators do not know what the crew was saying in the seconds leading to the crash.
The TSB found evidence that 12 similar accidents involving insufficient power on takeoff had resulted in 300 deaths around the world. Your workplace probably has nothing to do with airplanes, but rocket science isn’t required to see how fatigue can put your workers at risk.
